Association Between Cervical Discopathy and Temporomandibular Joint Dysfunction

    Taspinar, Ozgur

Objective: Cervical discopathy is a pathology resulting from the displacement of the nucleus pulposus into the cervical spinal canal. Temporomandibular joint disorder (TMJ) is characterized by pain and limited movement in the temporomandibular region. The purpose of this study was to investigate the relationship between symptoms of TMJ dysfunction and cervical discopathy. Materials and Methods: The study included patients diagnosed with cervical disc herniation who presented to our outpatient clinic, along with a control group consisting of patients from other outpatient departments without cervical spine pathology. Participants were evaluated based on demographic data, systemic diseases, trauma history, TMD symptoms, joint sounds, facial pain, teeth grinding, teeth clenching, yawning, mouth opening capacity, masseter pain, and preferred sleeping position. Results: A total of 90 individuals participated in the study: 45 with cervical disc herniation and 45 controls. There were no statistically significant differences between the groups in terms of age or body mass index (BMI) (p>0.05). However, a statistically significant association was found between cervical discopathy and masseter pain, denture use, trauma history, TMD symptoms, teeth grinding, and teeth clenching (p<0.05 for all). Conclusion: This study found significant associations between cervical discopathy and TMJ symptoms such as masseter pain, denture use, trauma, teeth grinding, and clenching. These findings are consistent with the literature. The jaw joint should be carefully evaluated in patients with cervical discopathy.

Introduction: Fibromyalgia syndrome (FMS) is a chronic disease characterized by widespread musculoskeletal pain andhypersensitivity at some points of the body. Our study aims to investigate whether there is a difference between male andfemale FMS patients and male and female healthy volunteers concerning dermatological findings. Methods: Sixty female and 60 male patients, who were diagnosed with FMS, were included in the study group; 60 femaleand 60 male healthy volunteers were included in the control group in this study. Fibromyalgia Impact Questionnaire andthe Beck Depression Inventory were applied to all participants at the time of admission. All participants were examined bya dermatologist, and dermatological diagnoses were recorded. The results of the patients in both groups were comparedand statistically analyzed. Results: In the examination of dermatological diagnoses, it was found that the incidence of lichen simplex chronicus wassignificantly higher in female FMS patients than in healthy female participants (p=0.006).Discussion and Conclusion: Based on the results of this study, our findings suggest that dermatological examination inpatients with FMS should be given more importance within a holistic examination approach.

The main purpose of our study is to investigate the efficacy of peloidotherapy applied at home in subacromial impingement syndrome and applied at the hospital. 73 female patients who were diagnosed with subacromial impingement syndrome were enrolled on one-sided shoulder. The patients were divided into two different groups, the treatment group (n = 42) and the control group (n = 31). Peloid therapy was performed in the Peloid therapy unit to direct contact with the skin in patients with treatment group. The patients in the control group applied the Peloid therapy at home with their own facilities. Both groups continued their routine medical treatment and home exercise programs. In the evaluation, DASH-T (Disabilities of the Arm, Shoulder and Hand) scores were used to assess the patient's shoulder functions..VAS (Visual Analogue Scale) was used for pain and health Assessment Questionnaire HAQ (Health Assessment Questionnaire) was used for general conditions. In the study group, in terms of DASH and HAQ scores, there was no significant difference after treatment (P > 0.05).In the study of Peloidotherapy in the study group, more positive results were obtained in pain parameters according to peloidotherapy applications in the control group. Studies are needed to increase the number of cases in the future.

Objective: Our open-label clinical trial; to investigate the effect of dry needling on sleep problems in fibromyalgia patients with trigger points. Material and Method: Informed consent forms were obtained from a total of 100 patients with trigger points in the upper trapezius and rhomboid muscles who were under regular medical treatment, and followed up with a diagnosis of fibromyalgia for at least six months in the outpatient clinic. All patients were evaluated with FIQ (Fibromyalgia Impact Questionnaire), BDI (Beck Depression Scale), PSQI (Pittsburg Sleep Quality Index) and VAS (Visual Analog Scale). The patients were divided into two groups by computer-assisted simple randomization. In the study group, 50 fibromyalgia patients with trigger points were given a total of three dry needle injections to the upper trapezius and rhomboid muscles for a total of three weeks at weekly intervals, while 50 fibromyalgia patients with trigger points in the control group were not given any needling. One month after the first evaluation questionnaires of all patients (all of the study and control groups), the FIQ, BDI, PSQI scores were checked again and the patients were questioned by VAS (Visuel Pain Scale). Results: There was a statistically significant difference in FIQ, PSQI and VAS scores when the groups were compared in terms of amounts of change (respectively; p<0.001, p=0.007 ve p=0.035). Conclusion: In our study, it was observed that dry needles applied to trigger points of fibromyalgia patients had a statistically significant contribution to patients in terms of FIQ, PSQI and VAS values.
